How To Fill JEE Main Self Declaration Form 2025?

JEE Main self declaration form 2025 is a mandatory document to appear for the examination. You must read the self declaration form carefully. The step-by-step process of filling JEE Main self declaration form is given below:

1. First, tick all the boxes regarding the covid-19 symptoms (if you have any). The symptoms may include fever, cough, sore throat, body ache and breathlessness.
2. Fill in your health history. (if any)
3. Tick the box if you have come in contact with any covid-19 patient.
4. Tick the box if you have been quarantined.
5. Fill in your travel history. This includes the cities that you have been to in the past 14 days. You should also specify the number of days you have stayed there.
6. Now, do signature in the provided space.
7. Put your left hand thumb impression in the given space and paste your passport size photograph.
8. Lastly, signatures of your parents are required in the space provided.

JEE Main self declaration form is submitted to the invigilator in the exam hall. You’ll only get the entry in the exam hall when you have your duly filled and signed JEE Main self declaration form.
