- NEET MDS admit card and ranking
- Class 10 and 12 mark sheets and certificates
- BDS degree certificate
- Completion certificate during internship
- Permanent Registration certificate through the DCI or State Dental Council
- Domicile certificate for state quota seats
- Category certificate- if applicable
- Photocopies of recent passport sized photographs
- All other documents sought by the counselling authority.
