- HSC mark Sheet
- MH-CET 2017 Score card
- Leaving Certificate
- Nationality Certificate
- Domicile Certificate
- Caste Certificate (for Reserved Category)
- Caste Validity Certificate (for Reserved Category)
- Non creamy layer certificate VALID UPTO 31/03/2018 (for OBC/SBC/NT/VJ/DT)
- Income proof certificate (for OBC/EBC)
- Two passport size photograph
- D. Pharmacy-I year mark sheet
- D. Pharmacy-II year mark sheet.
