10th Marksheet , Scheduled Caste/Tribe Certificate, 12th Marksheet ,Certificate regarding participation, School Leaving/ Migration Certificate in Sports at National/State Level( if any),Character Certificate , Certificate course ( if any) , Certificate of Physical Fitness in the are the documents required for the form filling . The copies of the documents must be self attested.
