Government Medical College, Kozhikode has released the list of documents for MD/MS Admissions 2024-25. Candidates must submit all the original documents at the time of admission. Further, students are advised to send a soft copy of the documents in PDF format to academicmcc2@gmail.com.

Documents Required

  1. Admit card issued by the NBE/CEE
  2. Rank Letter of NEET
  3. Provisional Allotment letter-(2 copies)
  4. Self attested copy of any Valid ID proof
  5. 10th Certificate
  6. 12th Certificate to be produced at the time of adm
  7. Nativity Proof (See Clause 4.2 of the Kerala NEET-PG Prospectus)
  8. MBBS Degree Certificate
  9. Mark lists of MBBS 1st 2nd and 3rd (part I and II)
  10. CRRI/Internship Certificate
  11. Kerala Medical Council Registration Certificate (KSMC) (Any Medical Council Registration Certificate will be enough at the time of admission. But they have to submit the KSMC Registration certificate within 1 month, if the candidate wishes to continue in this institution)
  12. Transfer Certificate (TC) (if the institution does not issue TC, a certificate from the institution mentioning that no TC will be issued from the institution)
  13. Conduct Certificate
  14. Proof of reservation, if allotted seat comes under reservation category.
  15. Passport Size Photo (2 Nos)
  16. Physical Fitness Certificate
  17. Migration Certificate (For candidates who have passed the qualifying examination from the universities other than KUHS, must submit within 2 months if the candidate continuous in this institution)
  18. Equilavance / Eligibility certificate from Kerala University of Health Sciences, Thrissur (for candidates who have passed the qualifying examination from the universities other than KUHS). Must submit months if the candidate continues in this institution) within 2 months if the candidate continues in this situation
  19. Bond (Candidates have to execute two bonds of Rs.200/-each. One bond to the effect that they shall serve the Government of Kerala for a minimum period of 1 year after completion of the course and the other for paying liquidated damages if they discontinue the course. Students are directed to get the bond paper printed from LRC. Bond should be submitted within 7 days from the date of admission.
  20. If the candidate has claimed any fee concession in NEET PG even if they are not allotted under Reservation quota, they must produce the certificate used for claiming the concession at the time of counseling.
  21. Students are directed to file an online affidavit at https://antiragging.in and have to submit the details at the time of Admission (Please see the details in college website). The email received by the student after the successful submission of anti ragging registration has to be forwarded to "antiragginggmck@gmail.com"
  22. Students are directed to send soft copy of all original documents separately in PDF format to academicmcc2@gmail.com
