MBBS in Japan means studying at one of the world's most technologically advanced healthcare systems — but almost entirely in Japanese, with only one university offering English-medium instruction. MBBS in Germany (called Medizin or Humanmedizin) is tuition-free at public universities, but demands C1-level German and one of the most competitive non-EU admission processes in the world.

What MBBS in Japan and Germany Actually Mean for Indian Students

Neither country awards an "MBBS" degree. Japan awards a Bachelor of Medicine (MD equivalent) after a 6-year program. Germany awards the Staatsexamen (State Examination in Medicine) after 6 years and 3 months, followed by the Approbation — the permanent medical license.

Both are recognised as equivalent to MBBS for NMC purposes. Both require Indian students to clear the FMGE (Foreign Medical Graduate Examination) or the upcoming NExT exam to practice medicine in India after graduation.

The Language Barrier: Which Country Is Actually Harder for Indian Students

Japan is significantly harder on the language front. German takes 12–18 months to reach B2/C1 for most Indian students. Japanese takes 3–5 years to reach JLPT N1 — the level required for clinical training and the Japanese Medical Licensing Exam.

Even at IUHW Narita — Japan's only English-medium medical school — the official curriculum states that from Year 3 onwards, all clinical training is conducted in Japanese. You will be diagnosing and treating Japanese-speaking patients. There is no workaround.

In Germany, the language barrier is real but more manageable. B2 gets you into Studienkolleg. C1 gets you through the clinical years. C2 is required for the Approbation in most German states. The timeline is demanding but achievable within 18 months for a dedicated learner.

Note for Indian students: Multiple students on Reddit's r/movingtojapan and r/mext have flagged that even after clearing JLPT N2, the medical Japanese used in clinical settings is a separate challenge. Medical terminology in Japanese is largely derived from Chinese characters (kanji) — not covered in standard JLPT preparation. Budget an additional 6–12 months for medical Japanese specifically.

Germany is significantly more affordable than Japan for MBBS. At German public universities, there is zero tuition. At Japan's national universities, tuition is JPY 535,800/year (INR 3.16 lakh/year). The cost gap widens further when you factor in living expenses. Important caveat on Japan national universities: National universities in Japan teach almost entirely in Japanese. The INR 63.5 lakh figure for national universities assumes you can gain admission — which requires JLPT N2/N1 and passing the EJU (Examination for Japanese University Admission for International Students). Admission Requirements: How Hard Is It to Get In?

Both countries have extremely competitive admission processes for Indian students. Germany reserves only ~5% of medical seats for non-EU students. Japan's IUHW Narita offers just 20 international student seats per year out of 140 total.

What does Japan require from Indian students?

For IUHW Narita (the English-medium option), the official admission requirements are:

  • Completion of 12 years of schooling outside Japan
  • Valid Student (Ryugaku) visa eligibility
  • English proficiency: TOEFL iBT 80+ or IELTS 6.0+ (as a reference guideline)
