CTM nurses work with patients and families to design, implement, and participate in care coordination projects and practices that improve patient outcomes and decrease costs. Leading by example, CTM nurses demonstrate the effectiveness of nurse-led and patient-centered care coordination. Nurses completing this program will learn how to:
- manage the transition of care from inpatient to outpatient settings
- help patients manage access to preventative, acute, and chronic health care services
- improve patients? and families? access to healthcare and quality of care
Classes are held on a monthly, weekend-intensive schedule.
